Job Summary:
The School Health Associate will assist the professional staff in the total implementation of the Health Services program.   This is a long term substitute, full-time position working at Pine Bend Elementary during the 2020-2021 school year.

Required Qualifications:

  • Graduate of accredited RN or LPN school.  Current RN or LPN license in the State of Minnesota.
  • An interest in working with children/adolescents.
  • Ability and willingness to learn screening procedures; vision, hearing, pediculosis, height and weight, and scoliosis.
  • Organizational skills.
  • Respect for confidentiality of information.
  • Good oral and written communication skills.
  • Ability to react quickly and decisively in a crisis.
  • Ability to function with a minimum of supervision.
  • Computer skills.
  • The School Health Associate will maintain current CPR certificates.


First Aid
Participate in emergency care of injured or ill students/staff according to district policy.
Complete accident reports for administration and for file in Health office.
Notify parent/guardian as indicated on the emergency card.
May not transport students.

Dispensing Medication
Medicine is not dispensed without instruction and written orders from the attending physician and parent permission.  See Minnesota School Health Guide.
Consult with Supervising Licensed School Nurse regarding dosage, schedule for administration, side effects and other pertinent data regarding medication.
Keep accurate records of medications brought to health office; and records each administration of medication.

Immunization Data Collection
Obtain complete immunization histories on all new or transfer students according to district policy.
Notify Supervising Licensed School Nurse of non-compliant student.

Prepare and maintain cumulative health records on all students.
Record appropriate data on school health records at the direction of the Supervising Licensed School Nurse.
Record all visits of students and informs Supervising Licensed School Nurse and Principal of unusual and/or serious pupil illness or injury, refer to Supervising Licensed School Nurse about exclusions or re-admissions.
Collect and maintain staff and student emergency cards.
Check absentee records and follows up on children who are frequently absent in coordination with attendance office and/or Supervising Licensed School Nurse.
Compile a list of health concerns and handicapped children with information that is current and records on their cumulative health record.
Complete all necessary records regarding Third Party Reimbursement for MN eligible students.

Perform initial screening procedures as directed by the Supervising Licensed School Nurse.
Assume responsibility for recruiting and scheduling volunteers for the screening program or uses chairperson from PTSA.
Schedule screening with Supervising Licensed School Nurse, Teachers and Principal.
Record screening on health record.
Schedule rechecks for students who failed 1st screening.
Keep record of all referrals sent.
Contact parents if referrals are not returned.
Prepare preliminary reports of screening for vision, hearing and scoliosis for State Department of Health for Supervising Licensed School Nurse.

Other Programs
Assist with dental health month in February.
Family Life:
Consult with grade 5 teachers and elementary school nurses to schedule films.
Assists as needed or directed by Supervising Licensed School Nurse.

Procedures for Students with Disabilities
Perform necessary nursing procedures as delegated by the Supervising Licensed School Nurse and at the direction of the Supervising Licensed School Nurse.
Assist in adaptations to allow the student to function in the school setting.

Communicate with Supervising Licensed School Nurse by phone/visit as necessary.
Communicate objective health data to student and teaching staff as appropriate.
Notify parents or guardians of illness or injury as needed.
Notify Supervising Licensed School Nurse and/or Principal of serious incidents including child abuse.
Attend health service staff meetings as scheduled.

Other Duties and Responsibilities
Assume responsibility for the cleanliness of the office.
Order supplies and forms from the district office as needed.
Take responsibility as delegated for exclusions and readmission of students according to school policy.
Alert to health and safety hazards in the school environment.
Carry out policies and procedures adopted by the school administrative body.
Maintain confidentiality of information learned or received on the job about students and their families.
Contact the parent/guardian for transportation of ill/injured students according to district policy (on student emergency sheet).       
Participate in special meetings as requested by Supervising Licensed School Nurse and/or Principal, i.e., meetings regarding students with special needs, and contributions to health component of the IEP.
Meet with Supervising Licensed School Nurse periodically.
Attend special workshops as requested, i.e., diabetes, scoliosis, vision, hearing and blood pressure screening.
Perform all other related work delegated or required to accomplish the objectives of the total school health program.
Serve as resource to staff for health education purposes.

Pay and Benefits:

The rate of pay for this position is $22.36 per hour.  Insurance benefits are available to full-time employees and part-time employees who work at least twenty hours per week.
